Quick answer: A semen analysis measures volume, sperm number, how sperm move, and how they are shaped. It is a lab snapshot, not a fertility verdict. The World Health Organization lower reference limits include about 1.4 ml volume, 16 million sperm per millilitre, 42 percent total motility, 30 percent progressive motility, and 4 percent normal forms. One abnormal report should be repeated and read by a doctor.
What each line usually means
- Volume. How much fluid was collected. A very low volume can mean a collection problem or a blockage question, not automatically a low sperm factory.
- Concentration and total count. Sperm per millilitre, and sperm in the whole sample. Total count matters as much as the per-millilitre figure.
- Motility. The share of sperm that move, and the share that move forward. Forward movement is what helps sperm reach the egg.
- Morphology. The share with a typical shape. The lower reference limit is only a few percent. A low morphology number on its own rarely tells the whole story.
- White cells or clumping. These can suggest infection or antibodies and need a clinical reading, not an antibiotic bought without a test.
Values above the reference limits are reassuring. Values below them are a reason to look further. Men with results inside the limits can still need help conceiving, and men slightly below them still conceive. The couple is the patient, not the single percentage.
How to give a useful sample
- Abstain for the number of days the lab states, commonly two to seven.
- Collect the full sample. Missing the first drops can lower the count.
- If collection is at home, deliver it within the time the lab sets, kept near body temperature, not in a hot car or a refrigerator.
- Tell the lab about fever, a new medicine, testosterone injections, or anabolic products. Outside testosterone can shut down sperm production.
When to repeat, and what happens next
If the first report is abnormal, repeat it after a few weeks. Sperm develop over about three months, so a recent illness can fade on the next test. At Orene Uro and Andro Care, Dr. B. Sourabh Chandra Reddy looks at both reports with the history: time trying, the partner’s age and evaluation, varicocele, infections, and medicines. Examination comes before a stack of hormone tests. Treatment may be lifestyle and timing advice, treatment of a correctable cause, or referral for assisted reproduction when that is the efficient path. More on local patterns is in male infertility in Hyderabad.
What not to do with the PDF
Do not start hormone tablets, herbal “sperm boosters,” or testosterone because a chat group shared a photo of their report. Testosterone prescribed without a fertility plan can drop the count further. Bring the PDF to an andrologist and decide from there.
Frequently asked questions
What is a normal sperm count?
The WHO lower reference limit for concentration is 16 million sperm per millilitre, with total number, movement, and shape also reported. A value near or below a limit is not a verdict of infertility. One report is interpreted with a repeat test and your history.
How many days of abstinence are needed?
Most labs ask for two to seven days without ejaculation. Shorter or much longer gaps change the numbers. Follow the lab sheet you were given.
Can fever or illness spoil a report?
Yes. A fever in the previous three months can lower count and movement because sperm take about that long to develop. Say so before anyone labels the result as a permanent problem.

Key takeaway
Read a semen analysis as a set of WHO reference limits, then repeat an abnormal test. In Hyderabad, an andrologist should interpret it with both partners in mind.
